Why can't I see my Medical Records after I register for MCPN Patient Portal?
Towards the end of registration if you received an error that Registration
couldn't be verified then you are able to view the portal but not
information that is Patient Health Information. Try to re-register
and when you get to the verification area make sure the Pin# given to you
is exact. Also be sure to accurately complete all required information
which is listed on the sheet given to you at the clinic. Once
information has been verified you will have access to your Medical Records.
Is MCPN Care Connection safe to use?
Yes. We protect your information with 256-bit Secure Socket Layer security.
Can you use MCPN Patient Portal to request lab results?
Yes. You can request lab results from your provider. This is an easy way to request results without having
to call into MCPN.
What if I have a medical emergency?
MCPN Patient Portal should not be used for urgent medical matters. If you have an urgent medical
problem, call 911 or your provider’s office right away.
Can I access my child’s medical record through MCPN Patient Portal?
Yes. If your child is a MCPN patient, you can sign up for proxy access to MCPN Patient Portal for
children 12 and under and you will have access to their health information. Ask a MCPN staff member for
more information on how to access your child’s health information.
Who can I call if I have problems accessing my chart?
MCPN wants your online experience to be as easy as possible. If you should have problems accessing
your health information online, ask a staff member for assistance or please call our Customer Care Center

What if I misplace my PIN?
Can I just call my location of care to have a new PIN assigned to me?
MCPN values your privacy and the confidentiality of your health information. If you lose your PIN for
the activation of your MCPN Patient Portal account, we ask that you come into one of our clinics in person
and request a new PIN from a staff member.
